'Negligence' in Zeyrek's death documented one by one! Why was the RCD missing?

An expert report has emerged in the investigation into the death of Manisa Metropolitan Municipality Mayor Ferdi Zeyrek, who lost his life after being electrocuted.
According to the report cited in Sözcü, serious electrical safety negligence was identified in the pool energy room where the death occurred. It was noted that circuit breakers, which should protect human life, failed to activate, and that there were errors in connections and significant wear on equipment.
It was emphasized that the wet floor reduced resistance, causing the fatal current to increase.
FAULTY CONNECTIONS IDENTIFIED
It was determined that there was moisture-induced corrosion in the area and on the bolts that caused the accident, and that there were faulty connections that did not comply with standard conditions.
All these issues were included in the report with photographs. Devices that protect human life against electric shocks were specifically tested. A 67-volt potential was measured between the metal ladder and the metal frame. During this measurement, it was determined that the circuit breaker devices known as RCD and MCB, which protect people against a potential electric shock, did not activate.
LEAKAGE IN MOTOR COVER
It was revealed that the inside of the supply point on the pool's pump motor cover contained moisture and, in some places, accumulated water. Engineers also shared that there were signs of burning here and that a burnt smell was coming from inside the cover.
It was also observed that the gasket between the cover on the motor supply point and the motor itself was damaged, making it susceptible to moisture and water ingress.
WHY WAS THERE NO CIRCUIT BREAKER?
The conclusion and opinion section of the report specifically included the following points:
It would be appropriate to investigate why the 30mA RCD, which should protect the circuits and is required to be at the apartment entrance, was missing, and if it was present before, by whom it was removed.
It would be appropriate to investigate whether there was any intentional external interference with the supply cover and internal windings on the motor.
Investigating whether the pool pump motor had undergone maintenance previously (in order to find the person who interfered with the motor).
It would be appropriate to investigate whether the installation in the panel inside the pool energy room had undergone acceptance and maintenance procedures, and whether a test procedure had been performed on the residual current device.
"MUST BE INVESTIGATED"
It would be appropriate to investigate whether a grounding measurement had been performed inside the pool energy room previously. (By the building inspection firm, private firms, etc.)
It was determined that the filter system mentioned at the scene, which was stated to have been installed a few days prior, only shared the same supply point and had no direct connection to the pool motor's cover, windings, or supply point. It would be appropriate to investigate who performed the last maintenance or interference on the motor before the incident occurred, on what date, and whether there were existing problems during that maintenance.
WET AREA CAUSED CURRENT TO INCREASE
It would be appropriate to determine who was responsible for the design, equipment selection, and installation of the pool section's energy room, which did not comply with standards.
Regarding Zeyrek's death, it was clearly stated in the report that the wetness of the area, as a result of the conditions described above, reduced the resistance and caused the calculated current to increase.
