Hair loss warning for GLP-1 weight loss drugs: Risk is low but higher compared to other medications

A new study on GLP-1 receptor agonists used in the treatment of type 2 diabetes and obesity has revealed that while the risk of hair loss is generally low for those using these drugs, it is significantly higher compared to some alternative diabetes medications.
In the study published in The BMJ, researchers compared the use of GLP-1 class drugs with other classes of diabetes medications, such as SGLT-2 inhibitors and DPP-4 inhibitors. The analysis, conducted via electronic health records, reported that GLP-1 receptor agonists increased the risk of alopecia by 37 percent compared to SGLT-2 inhibitors and by 68 percent compared to DPP-4 inhibitors.
Reports of hair loss associated with active ingredients such as semaglutide and tirzepatide had previously surfaced. While the new study shows that this effect is rare but cannot be entirely ignored, researchers stated that this risk information could help patients make more informed decisions before starting or continuing treatment.
One important point highlighted in the study is that most cases of hair loss associated with GLP-1 use are reversible. The authors stated that hair follicles generally remain intact, meaning the shedding may not represent a permanent loss.
WHY MIGHT THE RISK BE INCREASING?
According to the researchers, the mechanism behind the hair loss is not yet clear. However, rapid weight loss and a sharp reduction in caloric intake have long been known as factors that trigger hair loss. It is considered that caloric restriction can lead to deficiencies in micronutrients such as iron, zinc, and biotin, as well as physiological stress, which can disrupt the hair growth cycle.
The authors emphasized that hormonal changes might also play a role in this process, but further research is needed to understand the link between GLP-1 treatment, weight loss, and alopecia.
GLP-1 receptor agonists work by mimicking the effect of the glucagon-like peptide-1 hormone produced in the intestines after meals. These drugs stimulate receptors in the pancreas to support insulin secretion and help regulate appetite through receptors in the brain. Initially developed for the treatment of type 2 diabetes, GLP-1 drugs have over time begun to be used in different areas such as weight loss, reduction of cardiovascular risk, and obstructive sleep apnea.
Although the absolute level of clinically recorded alopecia risk associated with the use of GLP-1 receptor agonists is low, the observed association may still affect patient satisfaction, treatment adherence, and decisions to start or continue treatment.
Although hair loss does not often cause direct physical harm, researchers pointed out that it can have significant psycho-social effects on self-esteem, quality of life, and treatment adherence. The study concluded that further research is needed to identify patient groups at the highest risk and to clarify potential mechanisms.
