Chain of negligence in expert report regarding Ferdi Zeyrek's death: 4 people found primarily, 9 people secondarily at fault

A 46-page expert report prepared regarding the incident in which Ferdi Zeyrek lost his life has been entered into the case file. While 4 people were found primarily at fault and 9 people secondarily at fault in the report, it was stated that Zeyrek had no fault in the incident. Negligence in the electrical installation and practices contrary to the project drew attention.

The expert report prepared regarding the death of Manisa Metropolitan Municipality Mayor Ferdi Zeyrek in the pool machine room of his home on June 6, 2025, has been entered into the case file. In the 46-page report prepared by a 5-person committee including faculty members from Dokuz Eylül University Faculty of Engineering, 4 people were found primarily at fault and 9 people were found secondarily at fault. The report stated that Zeyrek had no fault in the incident.

According to the report submitted to the Manisa 2nd High Criminal Court, the swimming pool and machine room were not included in the initial license projects for Zeyrek's home. It was determined that the structure, which was shown as a 4-meter by 8-meter "ornamental pool" in the subsequent architectural renovation project, was built as a swimming pool with a depth of 160 centimeters.

According to the report by Cumhuriyet, the expert report stated that the building contractor M.G. covered and hid the pools and the machine room, which were contrary to the project, during the construction process, and reopened these areas after the occupancy permit was obtained. The report also included that municipal personnel and inspection authorities signed documents stating that the structure was completely in accordance with the projects while there was water in the pool.

CRITICAL NEGLIGENCE IN INSTALLATION

One of the most striking findings of the expert committee concerned the "Niobe Havuz" company, owned by N.B., which performed the electrical installation of the pools at the site. The report stated that the company's main field of activity appeared as "General Cleaning and Pest Control Activities" in the Trade Registry Gazette.

It was noted that N.B., who was stated not to be registered with the Chamber of Electrical Technicians, undertook the electrical work for 6 pools with a total power of 28.08 kW under a single contract, despite being legally authorized to perform installations with a maximum power of 16 kW.

The report determined that grounding and potential equalization projects were not prepared for the electrical installation. The expert committee reported that if grounding had been done in accordance with standards, the contact voltage that would occur on the human body would have remained at the 6-Volt level; due to the negligence, a voltage of 67 Volts occurred on the metal ladder that Zeyrek touched. It was also determined that the residual current relay was faulty.

It was stated that the 750-Watt pool jacuzzi motor, which was said to have caused the electrical leakage, was repaired by H.A. and H.İ. in 2022, but the water sealing gaskets were not installed during assembly and the fitting connections were left loose. The report also stated that the connections of the chlorine generator, which was installed a few days before the incident, were made incorrectly, thus creating a dangerous system that was impossible to operate.

The report also reflected that there was a puddle of water on the floor of the machine room on the day of the incident and that the electrical system was not working normally. The experts assessed that the use of the machine room should have been prevented until the electrical system was made safe, and that this situation was not reported to Ferdi Zeyrek.

The report included information that his wife, Nurcan Zeyrek, who tried to save Zeyrek, was also exposed to the leakage current; despite this, she managed to cut off the electricity of the facility by turning off the switch on the main house panel.
